• I understand that upon registering my child for any classes at IHSDC, that I assume all risks to my child and understand and agree on behalf of myself and my child, to release, hold harmless, and discharge CreativeOne Productions, Inc. d/b/a In His Steps Dance Company from all claims, costs, liabilities, expenses or judgements for any occurrences in connection with any instruction or activity.  I understand that IHSDC is not responsible for my child or other children under my supervision who are left unsupervised in the common areas and areas surrounding the facility and that IHSDC will only be supervising my child when he or she is participating in scheduled activities, programs, or instruction. I understand that IHSDC is not responsible for personal property that is lost, damaged, or stolen while I or my child is at or on IHSDC property. 

• · IHSDC may photograph your child participating in individual or group activities. Photographs may be used to illustrate the studios activities and to promote IHSDC through, but not limited to brochures, website, social media, or other mediums. You agree to release IHSDC from any expectation of confidentiality for your registered child and you attest that you are the parent or legal guardian of the child registered at IHSDC and that you have the authority to authorize IHSDC to use their photographs. 

• My registration is proof of my intention to execute a complete and unconditional waiver and release of all liability pursuant to the terms herein.  I am of lawful age and competent to accept this affirmation and agreement as to all listed terms and conditions. 

· Sibling Discounts:  1 sibling = $10 off, 2 or more siblings = $15 off of the total monthly family account tuition amount.  (Discount is per family account, not per student)

· A valid Credit or Debit Card must be saved on file.  Monthly payments are automatically debited on the 3rd of each calendar month.  If the primary card on file is declined, then any other saved cards on file are subject to incurring this charge. If tuition is not paid by the 15th of the month, your card on file will be charged for the monthly tuition along with a $20.00 late fee.  If tuition fees are not paid by the end of the month, then the student will be dropped from the class and not allowed to return to class until the account is paid in full. Tuition fees are the same monthly fee regardless of the student's attendance. 

· To drop any classes and cease any further monthly tuition charges, you must notify a studio office staff member by phone, via email, or in person prior to tuition charges being posted to your account on the 1st of any given month.

· Payments or credits will be posted to any outstanding past due balances prior to being posted to costumes or other merchandise purchases.

· A $30.00 charge will be assessed for any returned checks.

· 1-3 costumes per class (except for Acrobatics) will be required for students to participate in our dance recitals. Costume prices average around $100 per costume (much less for boys).  A $75 deposit per costume ($25 deposit for boys) must be paid in full (by October 1st for the Christmas Recital and by January 15th for the end of the year recital) before costumes will be ordered for the student.  The balance of the costumes must be paid in full before the student will receive his/her costumes.

· For the end of the year recital, a $50 Recital Fee must be paid by May 31st.  This fee will include a professional recital video download, a recital t-shirt with the student’s name, and a medal for each student. 

· Students will not be allowed to perform in the end of the year recital if their dance account is not paid in full prior to the recital date.

· Private Lessons are scheduled directly with the teacher and are based on their schedule and discretion.  Private Lessons must be cancelled at least 4 hours prior to your appointment time or the full private lesson fee will be charged to the student regardless of the student's attendance.

· Proper dance attire is required:  Leotard, tights, and the appropriate dance shoes for the selected classes.  Absolutely NO street clothes.  Hair must be pulled back and out of the face.
